All Coastal Health District County Health Department employees are deemed essential employees in the event of an emergency. As such, you may be called on to participate in an event not limited to but including, staffing a shelter, a Specimen Point of Collection Site (SPOC) and/or Point of Dispensing site (POD). When you are called on to participate, you are expected and it will be a part of your job requirement to participate, whether the emergency occurs during or outside regular work hours.

Job Responsibilities

With supervision, assists with community outreach & engagement among at-risk populations. Provides support and training for community members on resiliency and self-empowerment, regarding accessing public health services. Connects at-risk populations to community public health services and services outside of public health (i.e., medication assistance, housing, transportation, education, etc.).  Provides follow up communication with referrals for hypertension and diabetes prevention and/or control, identified at health screening outreach events. With information gathered from the Health Equity community needs assessments, seeks out internal and external community partnerships to meet those needs. Provide data management and support for reporting and data analysis. Assists the Health Equity Coordinator with identifying community partners, partnering opportunities/community events and creating/maintaining the Health Equity Team’s community outreach calendar.

Minimum Qualifications

Bachelor's degree in education, social, behavioral, or biological sciences or a directly related field from an accredited college or university.
