Nature of Work

The Coconino County Health & Wellness Clinic is seeking a motivated Nurse who is passionate about Community Health and serving our community. This position utilizes standing orders from our medical director to deliver immunization and wellness services throughout the lifespan. With a great deal of autonomy afforded to this position, the ideal candidate will have an innate ability to interview, assess, and educate community members of all ages and backgrounds. Through collaboration with community partners, this individual will also work with the wellness team to plan and execute educational opportunities to engage with members of our community that may have decreased knowledge of and/or access to vaccines and wellness education. This position would follow standard clinic operating hours, with occasional off-site events during off-hours. The ideal candidate will be a nursing professional who has a passion for creating a healthy community, and experience with delivering education, as well as a toolbox of strategies to reach our very diverse population. Under general supervision performs professional nursing care of considerable difficulty for clients with complex health care needs; performs related work as assigned.  
 
Please note: Salary offers are based on the candidate’s equivalent experience and internal equity with other employees within the same job classification. RNs with a bachelor’s degree in nursing are eligible for an hourly pay differential above equally qualified candidates with an associate’s degree in nursing.
 
 Community Nurse I starting range: $27.46 to $28.85 per hour
 Community Nurse II starting range: $29.60 to $31.06 per hour

Typical Duties

  • Implements specialty public health programs focused on decreasing the incidence and spread of vaccine-preventable diseases.
  • Implements the nursing process which includes physical assessment, nurse diagnosis, interventions, and evaluations with clients
  • Provides education and counseling as needed by the individual
  • Networks with and refers to community resources appropriate to client's need
  • Administers immunizations and prescribed treatments according to standing orders or individual physician's orders
  • Participates in prevention and control of communicable diseases
  • Performs clinical nursing duties in immunization administration and tuberculosis testing, as well carries out home visits to our homebound seniors to deliver immunizations.
  • Identifies and networks with a variety of community resources to address client needs
  • Participates in orientation and clinical supervision of nurse aides and student nurses
  • Prepares and maintains individual patient records and program reports
  • May plan, implement, and monitor contracts, standing orders, budgets, quality assurance and standard of care policies and procedures

Essential functions of this position include but are not limited to: sitting for computer work; working with and around other staff and outside agencies; dealing with interruptions; reaching; repetitive motion (hand/wrist) for computer work, writing, and using a calculator; vision acuity (near/ far) must be able to see documents needing attention and driving; color vision for reading labels; hearing and speech for ordinary conversation with staff and customers; finger dexterity for typing/ 10-key.

Minimum Qualifications

Community Health Nurse I- Valid, unrestricted license as a Registered Nurse issued by the Arizona State Board of Nursing or issued by another state recognized by the Arizona Board of Nursing as a multi-compact site. 
 
Community Health Nurse II- Valid, unrestricted license as a Registered Nurse issued by the Arizona State Board of Nursing or issued by another state recognized by the Arizona Board of Nursing as a multi-compact site AND 2 or more years of experience. 
 
ADDITIONAL REQUIREMENTS:
 Must possess a current CPR certification.
 Must possess a current fingerprint clearance card issued by the Arizona Department of Public Safety (or the ability to obtain one).
 Must obtain an Arizona driver's license by date of hire and maintain it throughout employment.
 
Preferred Qualifications:

  • Understanding of immunizations and their effectiveness in creating community immunity and promoting wellness. 
  • Previous experience working in a clinic setting as a medical professional. 
  • Previous experience/knowledge of the Vaccines for Children state immunization program.
  • Experience working with the Arizona State Immunization Information System database. 
  • Bilingual.
