PAY AND BENEFITS
Exceptional Needs Care Coordinator (Classified as a Mental Health Services Coordinator)
Annual Pay Range: $73,699.88 - $93,581.75
Hourly Pay Range: $35.432637 - $44.991225
Salary offers will be made within the posted pay range and will be based on a candidate's experience (paid or unpaid) that is directly relevant to the position.

The Behavioral Health Division provides licensure supervision to all regular status employees who have completed a Graduate degree in a behavioral science-related field (social work, counseling, clinical psychology, etc.), and is actively pursuing licensure in that field.
There may be options for loan forgiveness while working with a government/public agency with the Public Loan Forgiveness (PSLF) program.

JOB DETAILS AND QUALIFICATIONS

Clackamas County Behavioral Health is seeking a Mental Health Service Coordinator, working under the title of Exceptional Needs Care Coordinator: Stabilization Specialist, to join our Severe and Persistent Mental Illness (SPMI) Care Coordination Adult Specialty Services Team.
In this specialized role, you will provide short-term care coordination and stabilization support to individuals with complex needs. Rather than managing a traditional caseload, this position partners with other Exceptional Needs Care Coordinators to support individuals requiring particularly intensive or time-sensitive behavioral health services and community support.
This position plays a critical role in supporting individuals who are experiencing severe mental health illness and are at high risk for civil commitment by helping them stabilize in the community and avoid higher levels of care. The Exceptional Needs Care Coordinator: Stabilization Specialist provides focused, short-term care coordination and resource linkage for individuals with complex behavioral health needs. Through collaboration with service providers, housing supports, and mental health networks, this role helps ensure that individuals receive timely access to resources and services that promote recovery, safety, and long-term stability. This is a unique opportunity to apply your clinical skills in a dynamic, community-centered role that intersects law, health, and social justice.
The ideal candidate will bring a strong clinical foundation, excellent collaboration and communication skills, and experience working with adults who have complex behavioral health needs. You are solution-focused, resourceful, and committed to helping people stabilize and remain in the least restrictive setting possible. Familiarity with housing, crisis, and outpatient systems, as well as multi-system coordination, is highly valued.

Required Minimum Qualifications/ Transferrable Skills:*
• Registered as a Qualified Mental Health Associate (QMHA) as established by the Mental Health and Addiction Counseling Board of Oregon (MHACBO) and/or State of Oregon.
• A Qualified Mental Health Associate (QMHA) means a person delivering services under the direct supervision of a Qualified Mental Health Profession (QMHP) and meeting the following minimum qualifications as authorized by the Local Mental Health Authority (LMHA) or designee:
(a) Bachelor's degree in a behavioral sciences field; or
(b) A combination of at least three years relevant work, education, training or experience.
• Minimum of three (3) years relevant experience demonstrating the required skills and knowledge to perform the duties of this position
• A minimum of one (1) year providing direct behavioral health services to individuals with Severe and Persistent Mental Illness (SPMI)
Preferred Special Qualifications/ Transferrable Skills:*
• Fluent in Spanish/English, both oral and written
• Experience working collaboratively with law enforcement, community partners, collateral service providers, and co-workers
• Knowledge of mental health, substance abuse, legal, medical, social, and housing resources in the community
• Experience as a Qualified Mental Health Professional (QMHP) providing direct experience with assessment, triage, and crisis intervention with individuals who are in mental health crisis
• Experience providing behavioral health support services in a community based setting.

TYPICAL TASKS
Duties may include but are not limited to the following:
• Provide short-term, community-based care coordination to individuals at high risk of psychiatric hospitalization, service disruption, or housing instability.
• Collaborate with Exceptional Needs Care Coordinators (ENCCs) to develop and implement individualized service and safety plans.
• Refer individuals to appropriate behavioral health services and monitor follow-through to ensure coordinated care.
• Coordinate with healthcare providers, social service agencies, and community partners to connect individuals with needed support.
• Advocate for client needs and ensure clear communication among service providers, clients, and families.
• Support smooth transitions across service settings by maintaining regular communication with outpatient and intensive service providers.
• Complete necessary documentation and participate in administrative tasks, including team meetings and case consultations.
WORK SCHEDULE
This position is included in the County's alternate workweek program, working 40 hours during a standard workweek of Tuesday through Friday (Mondays off).
Specific hours of work will be discussed with the candidate selected for this position at the time an offer of employment is extended.

The exact amount of on-site/telework and schedule will be discussed with the hiring manager during time of offer. Telework locations cannot be located outside of Oregon or Washington.
