PAY AND BENEFITS
Annual Pay Range: $65,833.47 - $83,420.32
Hourly Pay Range: $31.650705 - $40.105921
Salary offers will be made within the posted pay range and will be based on a candidate's experience (paid or unpaid) that is directly relevant to the position. This position is classified as a Human Services Coordinator 2.

JOB DETAILS AND QUALIFICATIONS
Clackamas County's Housing & Community Development Division seeks a dedicated and experienced Healthcare & Housing Services Coordinator. Your role will be to partner with Community-Based Organizations (CBOs), Coordinated Care Organizations (CCOs), health plans, long-term services and supports, and other healthcare service partners to deliver tailored housing and healthcare solutions. This focus on tailored solutions underscores our commitment to providing care that meets the unique needs of our community.
In this role, you will be pivotal in facilitating referrals for specific services, managing population-specific contracts tailored to support the housing needs of people with intellectual and/or developmental disabilities, those who need behavioral health or aging services with CBOs, and serving as a critical contact for health plan care coordination. This position requires a strong understanding of the housing and healthcare systems and the ability to collaborate effectively with various partners to meet the diverse needs of the population we serve.
As a Healthcare & Housing Coordinator, you will be at the forefront of connecting healthcare and housing services to individuals who need them the most. Your work will have a direct and significant impact on the lives of our community members. You will work closely with CCOs and their health plan partners, long-term services, and support partners, such as Aging and People with Disabilities and CBOs, to develop and implement policies that streamline referrals for essential services. By gathering information from the housing and healthcare systems, you will identify specific needs and facilitate effective case conferencing and care coordination meetings with health plan partners and behavioral health teams. This position will also carry a caseload of people with the highest needs to ensure recommended action for resources and referral is taken. This includes tracking recommended action, facilitating referrals, building connections with frequent referral entities, and ensuring recommended referrals or resources are provided. You will be the primary contact for care coordination, providing crucial resources and referrals to CBOs tailored to individual needs, such as accessing behavioral health housing navigation and case management. Your role will also oversee contracts with CBOs to ensure compliance and support population-specific services, including developmental disabilities, aging and people with disabilities, behavioral health, and corrections. You will participate in system integration meetings, representing our organization and providing insights to enhance collaboration and care coordination efforts. As a healthcare expert, you will educate health plan partners about available housing services and support policy and resource development to empower CBOs in understanding and accessing care coordination.
The ideal Healthcare & Housing Coordinator candidate is a compassionate and innovative professional with a deep understanding of healthcare and housing systems. They have a proven track record of successfully managing care coordination and contracts with Community-Based Organizations and building strong relationships with diverse stakeholders. They should also have a history with services coordination or complex case management, as some of this work will include managing a small caseload of people with high acuity needs to provide care coordination. Some of this work includes developing and overseeing person-centered housing plans. Their excellent communication skills and cultural competency allow them to connect individuals to tailored services effectively. At the same time, their analytical abilities and project management experience enable them to develop creative solutions that enhance service delivery. With a background in integrated care models and a passion for helping vulnerable populations, they are committed to positively impacting the community.
Required Minimum Qualifications/ Transferrable Skills:*
• At least four (4) years of experience of in care coordination, case management, or a related role in housing or healthcare system housing program (programs that include a housing subsidy and support services)
• Demonstrate knowledge of housing services, healthcare systems, and community-based organizations
• Strong communication and interpersonal skills to effectively collaborate with diverse partners and stakeholders
• Ability to establish and maintain effective working relationships with CBOs, mainstream and culturally specific supportive services in the community, and other public and private agencies and County employees
Preferred Special Qualifications/ Transferrable Skills:*
• Experience with Integrated Care Models
• Experience managing contracts and ensure compliance with contractual requirements
• Experience in policy development and resource management
• Knowledge of Medicaid, Coordinated Care Organizations, the Oregon Health Authority, and healthcare provide
• Knowledge of Long Term Services and Supports and Medicaid services
• Ability to understand regulations and use plain language to develop documentation and/or framework for documentation
Pre-Employment Requirements:

• Driving is required for County business on a regular basis or to accomplish work.
