PAY AND BENEFITS
Hourly Pay Range: $44.008292 - $55.853432
Salary offers will be made within the posted pay range and will be based on a candidate's experience (paid or unpaid) that is directly relevant to the position.

JOB DETAILS AND QUALIFICATIONS
The Clackamas County Public Health Division is seeking a dedicated Public Health Nurse for our Home Visiting program. This role involves providing in-home care coordination, education, and health navigation for families, including pregnant individuals, young children, and children with special health needs. The position requires collaboration with local and state partners to ensure the highest quality of care and connection to resources. Responsibilities include conducting assessments, documenting in EHR, and assigning work to Community Health Workers. The role also involves case management and care coordination for complex medical and social needs, delivering culturally sensitive nurse home visiting services to pregnant individuals and children with qualifying risk factors and medical needs.
Required Minimum Qualifications/ Transferrable Skills:*
• Possession of a license to practice as a registered professional nurse in the State of Oregon.
• Basic Life Support (BLS) certificate.
Preferred Special Qualifications/ Transferrable Skills:*
• Experience assigning and/or directing tasks of others.
• Epic electronic health records system experience.
• Experience providing maternal and child health nurse case management, including nursing assessments and interventions.
• Knowledge of utilization of quantitative and qualitative data for community health assessments and strategic planning initiates.
• Experience conducting case conferencing and reflective supervision.
• Experience building and maintaining caseload including working with the community to maintain a referral network.
• Fluency in both English and Spanish is preferred. (Please note: Language fluency means being capable of using a language easily and accurately. Being "bilingual" means using or able to use two languages with equal fluency.)

TYPICAL TASKS
Duties may include but are not limited to the following:
• Conduct comprehensive health and psychosocial assessments
• Develop and implement nursing care plans
• Provide assessments primarily in client homes
• Perform targeted case management assessments annually or sooner if needed
• Provide health education and outreach to families, individuals, and community groups on:
• Pregnancy, labor and delivery, postpartum, newborn, infancy, early childhood
• Medication and vaccinations
• Health system navigation for children with complex physical or mental health needs
• Assign and oversee the work of community health workers, including:
• Attending visits at least every 3 months
• Meeting monthly for case conferencing
• Signing off on charts and directing care plans to meet family needs and goals
• Make, coordinate, and follow up on referrals to medical, social, financial, and mental health agencies to meet clients' needs
• Identify and solve barriers to accessing referrals
• Coordinate with other health care workers to provide services and ensure continuity of patient care across settings (home, work, hospitals, clinics, and schools)
• Work with providers to improve communication and coordination for optimal client outcomes
• Assist clients with complex multi-system navigation, including health, social, psychological, education, and community settings
• Provide instruction and information on disease, medical conditions, health maintenance, prevention, and rehabilitation
• Participate in reflective practice individually and as part of a group
REQUIRED KNOWLEDGE AND SKILLS
Thorough knowledge of:
• Principles, methods, and procedures of nursing in health practice
• Preventative medicine and sanitation
• Community health aspects of nursing programs, including continuity of patient care, school health programs, and home care
• Causes, treatment, prevention, and early detection of communicable diseases, chronic diseases, disabilities, mental illness
• Sociological, psychological, and physical issues in public health nursing
• Principles of mental health
Working knowledge of:
• Community resources for public health and social services programs
• Current trends and concepts in professional nursing practice
• Office equipment, personal computers, and software programs
Skill to:
• Implement nursing activities in a district, program, or clinic under general direction
• Assist individuals and families in solving health problems effectively
• Analyze situations accurately and take effective action
• Establish and maintain effective working relationships with other departments, agencies, and the public
• Communicate effectively, both orally and in writing, including preparing clear records and reports
• Recognize social problems affecting physical and mental health and assist in finding solutions
• Provide high-quality nursing care
• Plan and coordinate activities of others in the absence of the Community Health Nurse
• Instruct in disease prevention, health promotion, and nursing care of ill or injured individuals
• Operate computer software and other office equipment
WORKING CONDITIONS
• This position involves exposure to bodily fluids and routine contact with individuals who have communicable diseases.
• This position requires the ability to drive personal vehicle throughout Clackamas County and provide face-to-face services within the community.
WORK SCHEDULE
This position works between 37.5 - 40 hours during a workweek of Monday through Thursday or Monday through Friday. Specific hours of work will be discussed with the candidate selected for this position at the time an offer of employment is extended.

The exact amount of on-site/telework and schedule will be discussed with the hiring manager during time of offer. Telework locations cannot be located outside of Oregon or Washington.
