PAY AND BENEFITS
Case Manager Compensation Range:
Annual Pay Range: $57,351.80 - $72,266.58
Hourly Pay Range: $27.572979 - $34.743549
Salary offers will be made within the posted pay range and will be based on a candidate's experience (paid or unpaid) that is directly relevant to the position.

JOB DETAILS AND QUALIFICATIONS
Clackamas County's Health Centers is seeking a Case Manager to join and support the Integrated Behavioral Health Team at our Beavercreek and Sunnyside clinic locations. The Case Manager performs skills training, case management, care coordination, and community-based support for individuals with mental health diagnoses or co-occurring mental health and substance abuse problems within our integrated behavioral health program. The Case Manager also works with family and community partners to provide education and coordinate access to resources, benefits and supports. The Case Manager in this position must be skillful in interacting and engaging with a diverse client base while maintaining professionalism in a high needs environment.
Successful candidates will be able to provide culturally competent services, have an anti-racist lens, will have completed some personal reflection and growth in their own equity journey, have a strong commitment to providing equitable services to the community, and demonstrating sensitivity to and respect for the diverse populations we serve. Case managers provide services to vulnerable individuals, many of whom may have limited resources. Professionals in these positions should thrive in a setting where they will work independently as well as in a team environment.

The person in this position must meet the requirements of a Qualified Mental Health Associate (QMHA) as outlined in the Oregon Administrative Rule and below.
Required Minimum Qualifications/ Transferrable Skills:*
• Qualified Mental Health Associate (QMHA) or (QMHA- R) as established by the Mental Health and Addiction Counseling Board of Oregon (MHACBO) at the time of hire.
A Qualified Mental Health Associate (QMHA) means a person delivering services under the direct supervision of a Qualified Mental Health Profession (QMHP) and meeting the following minimum qualifications as authorized by the Local Mental Health Authority (LMHA) or designee:
(a) Bachelor's degree in a behavioral sciences field; or
(b) A combination of at least three (3) years relevant work, education, training or experience.
• A minimum of three (3) years of related experience that would provide the required knowledge and skills to perform the responsibilities of this position
• Must possess and maintain a Basic Life Support (BLS) certificate
• Experience working in a primary care or behavioral health care setting
• Experience, knowledge and commitment to serving a diverse population and promoting equity, diversity and inclusion
Preferred Special Qualifications/ Transferrable Skills:*
• Fluency in Spanish/English, both oral and written (bilingual skills to be validated/tested at time of interview and/or selection)
• Experience providing case management services for clients diagnosed with severe and persistent mental illness (SPMI) and/or substance use disorders (SUD)
