The NCSB is seeking a Support Coordinator Trainee (Case Manager I) in their Developmental and Youth Services division. The Support Coordinator Trainee is responsible for participating in a curriculum of training and field experience under the supervision of qualified ID/DD Support Coordinators. Upon successful completion of this program the Support Coordinator Trainee will have attained the necessary knowledge, skills, and abilities required for ID/DD Support Coordinators in Virginia.Department Hiring Salary Range: $45,444-$52,148
Essential functions include but are not limited to:
- Completes initial and annual assessment of individuals for planning services and supports.
- Develops Individual Support Plan based on assessment information in collaboration with the individual and/or family.
- Provides linkage, coordination, and monitoring of services/supports to include Medicaid Waiver Services provided by private providers, other community agencies, natural support systems and other units in the NCSB.
- Maintains chart documentation that is in compliance with agency guidelines and state regulations. Ensures that documentation substantiates billing for services.
- Conducts face to face visits at a minimum of every 90 days and completes monthly activity related to the individual service plan.
- Monitors progress, service delivery, and individual satisfaction by providing monthly service to all individuals on assigned caseload. Provides increased frequency of service whenever clinically indicated.
- Identifies and accesses resources to meet identified needs and assist individuals in procuring and maintaining resources and entitlements.
- Assists individual/family in obtaining needed services on an emergency basis, i.e. hospitalization, housing, food, medications, etc.
Bachelor's degree in a human services field, or equivalent combination of training and experience.
No experience required.
- Valid Driver's License may be required depending on assignment.
- Certification in basic first aid and CPR, depending on area of assignment.
