Annual Pay Range: $57,351.80 - $72,266.58
Hourly Pay Range: $27.572979 - $34.743549
Bilingual Spanish/English skills are required for this position. An additional 5% of base compensation is added to pay.
Salary offers will be made within the posted pay range and will be based on a candidate's experience (paid or unpaid) that is directly relevant to the position.

JOB DETAILS AND QUALIFICATIONS
Clackamas County Health Centers is seeking a Bilingual (Spanish) Case Manager to provide skills training, case management, care coordination, and medication education for individuals with mental health conditions or co-occurring mental health and substance use disorders. The Case Manager works with families and community partners to provide education and coordinates access to resources, benefits, and support.
This position will work within the Diabetes Integration Program (DIP) providing services and support to adults diagnosed with diabetes or pre-diabetes. DIP has provided integrated physical, dental, and behavioral health care for Clackamas County Health Centers' patients with diagnoses of diabetes and severe mental health. This bilingual DIP case manager position will expand services to under-served Spanish speaking communities who experience diabetes at greater rates than majority communities.
Specific case management focuses may include, but are not limited to, providing group, individual, and family services identified in the primary care and/or mental health treatment plan; offering skills training, case management, and care coordination; supporting individuals diagnosed with Severe and Persistent Mental Illness (SPMI) and diabetes; and conducting outreach as indicated in the client's care plan. The Case Manager must be skilled in engaging with a diverse client base while maintaining professionalism in a high-need environment.

The applicant selected for this position must meet the requirements of a Qualified Mental Health Associate (QMHA) as outlined in the Oregon Administrative Rules, listed below.
Required Minimum Qualifications/ Transferrable Skills:*
• Qualified Mental Health Associate (QMHA) or (QMHA- R) as established by the Mental Health and Addiction Counseling Board of Oregon (MHACBO) as mandated by Oregan Administrative Rule at the time of hire.
A Qualified Mental Health Associate (QMHA) means a person delivering services under the direct supervision of a Qualified Mental Health Profession (QMHP) and meeting the following minimum qualifications as authorized by the Local Mental Health Authority (LMHA) or designee:
(a) Bachelor's degree in a behavioral sciences field; or
(b) A combination of at least three (3) years relevant work, education, training or experience.
• A minimum of three (3) years of related experience that would provide the required knowledge and skills to perform the responsibilities of this position
• Fluency in Spanish/English, both oral and written (bilingual skills to be validated/tested at time of interview and/or selection)
• Must possess and maintain a Basic Life Support (BLS) certificate
• Experience providing case management services to clients diagnosed with diabetes, severe and persistent mental illness (SPMI), and/or substance use disorders (SUD)
• Experience, knowledge, and commitment to serving a diverse population and promoting equity, diversity and inclusion
Preferred Special Qualifications/ Transferrable Skills:*
• Experience working in a primary care or behavioral health care setting
• Experience providing services and/or support to vulnerable clients

TYPICAL TASKS
• Provide group, individual, and family services as appropriate and identified in the Primary Care and/or Mental Health treatment plan: Actively engage clients in the most appropriate setting (home, office, community). Provide outreach to clients for missed appointments. Utilize agency-approved Evidence-Based Practices. Utilize outcome measures identified by Clackamas County Behavioral Health Centers (CCBHC) to monitor client progress in their recovery. Make mandatory reports as defined by state law and agency policy.Provide case management, skills training, and care coordination services for individuals who are diagnosed with SPMI and diabetes. Provide outreach as indicated in the client's care plan. Develop and coordinate resources to provide clients and potential clients with needed services, community outreach, and coordination with service providers.
• Coordinate with health care providers, social service personnel, and other agencies to obtain support and services; provide information and referral services to clients and families; reevaluate and update case plans and client eligibility on a regular basis; advocate for clients and other agencies for services.
• Provide individual/family skills training as appropriate and identified in the treatment plan on the following activities of daily living: safety planning, health practices, communication, time management, money management, nutrition, mental health symptoms, medication management, problem solving, family relationships, alcohol/drug use, leisure, community resources, social network, sexuality, productivity, coping skills, behavior norms, personal hygiene, grooming, and dress.
• Determine health care and social service needs of clients; interview applicants, recipients, relatives, guardians, care providers, and other involved parties; conduct comprehensive needs assessments, including psychosocial, financial, and physical factors; evaluate functional impairment of clients.
• Provide other duties as assigned.
WORK SCHEDULE
This position works 40 hours during a standard workweek of Monday through Friday.
Specific hours of work will be discussed with the candidate selected for this position at the time an offer of employment is extended.

The exact amount of on-site/telework and schedule will be discussed with the hiring manager during time of offer. Telework locations cannot be located outside of Oregon or Washington. Washington residents must perform 50% of their time on-site at Clackamas County.
*Note: For purposes of this recruitment the physical work location for this position is in transition of being relocated to Lake Road in Milwaukie Oregon. Currently the building is under construction. While under construction this incumbent selected for this position will work a 100% telework/hybrid schedule but only until construction has completed and the relocation has been finalized. At that time the exact amount of on-site/telework schedule will be discussed with the hiring manager to accommodate business needs. Our new location is scheduled to open later in 2024.
