PAY AND BENEFITS
Annual Pay Range: $68,868.87 - $87,447.48
Hourly Pay Range: $33.110035 - $42.042059
Bilingual Spanish/English skills are required for this position. An additional 5% of base compensation is added to pay.
Salary offers will be made within the posted pay range and will be based on a candidate's experience (paid or unpaid) that is directly relevant to the position.

The Behavioral Health Division provides licensure supervision to all regular status employees who have completed a Graduate degree in a behavioral science-related field (social work, counseling, clinical psychology, etc.), and is actively pursuing licensure in that field.
There may be options for loan forgiveness while working with a government/public agency with the Public Loan Forgiveness (PSLF) program.

JOB DETAILS AND QUALIFICATIONS
Clackamas County's Behavioral Health Division is looking for a dedicated, self-motivated Mental Health Services Coordinator to join our Intensive Care Coordination Team. In this role, you will coordinate care for adult and youth members with complex needs requiring a high level of care. As a Mental Health Services Coordinator (Intensive Care Coordinator), you will provide critical services by meeting with members, their representatives, care providers, and other support providers—whether in the community, virtually, or by phone. You will conduct ICC Needs Assessments, co-develop ICC care plans with members and their care teams, facilitate interdisciplinary meetings, and offer ongoing support, education, and guidance to help members achieve their goals.
The ideal candidate will have strong interpersonal skills and a proven ability to foster positive working relationships with colleagues, clients, families, care providers, community resources, and partner agencies. Experience in adult and youth services, including working with individuals affected by Substance Use Disorder (SUD), is essential. We are especially interested in candidates who can provide culturally competent care coordination and are committed to equity and personal growth in diversity and inclusion. Required Minimum Qualifications/ Transferrable Skills:*
• Minimum of three (3) years relevant experience that demonstrates the required skills and knowledge to perform the duties of this position.
• Qualified Mental Health Associate (QMHA) as established by the Mental Health and Addiction Counseling Board of Oregon (MHACBO)
A Qualified Mental Health Associate (QMHA) means a person delivering services under the direct supervision of a Qualified Mental Health Profession (QMHP) and meeting the following minimum qualifications as authorized by the Local Mental Health Authority (LMHA) or designee:
(a) Bachelor's degree in a behavioral sciences field; or
(b) A combination of at least three years relevant work, education, training or experience.
Additionally, QMHAs must demonstrate the ability to communicate effectively, understand mental health assessment, treatment and service terminology and apply each of these concepts, implement skills development strategies, and identify, implement, and coordinate the services and supports identified in an ISSP.
• Fluency in Spanish/English (bilingual skills to be validated/tested at time of interview and/or selection)
• Experience working directly with Adults with Severe and Persistent Mental Illness (SPMI)
• Experience working directly with youth with emotional, mental and behavioral problems
• Experience with acute mental health care (E.g.: Emergency Department, inpatient care, crisis services)
• Experience with electronic medical records
• Experience in utilizing community resources to meet mental health care needs
• Experience, knowledge and commitment to serving a diverse population and promoting equity, diversity and inclusion
• Ability to establish and maintain cooperative working relationships with colleagues, clients, families, care providers, community resources and other agencies
Preferred Special Qualifications/ Transferrable Skills:*

• Familiarity with relevant Oregon Administrative Rules and statutes

TYPICAL TASKS
Intensive Care Coordination:
• Conduct timely outreach and engagement with members, their representatives, and care providers, meeting in the community, by phone, or through virtual platforms.
• Perform ICC Needs Assessments and collaboratively develop Individualized Care Coordination Plans (ICCP) with members and care teams.
• Arrange, facilitate, or participate in monthly multidisciplinary care team meetings, using a person-centered approach whenever possible.
• Provide ongoing support, education, and guidance to help members achieve ICCP goals, and assist in accessing necessary services and resources.
ICC Screening and Coordination:
• Manage and monitor daily reports and emails for ICC referrals, completing all screenings within required timeframes.
• Coordinate with service providers for member care, including discharge planning with facility staff and notifying primary care providers as needed.
Administrative Duties:
• Maintain accurate and timely documentation across required databases, generate authorizations, and track information in various systems.
• Address Oregon Health Plan (OHP) enrollment issues for assigned clients.
• Develop and implement effective discharge plans to ensure ongoing mental health needs are met in integrated, least restrictive settings.
Community and Agency Collaboration:
• Collaborate with behavioral health providers, community agencies, schools, DHS, APD, and other partners to ensure smooth transitions and effective service delivery across settings.
General Office Administration:
• Perform routine office tasks, including copying, printing, faxing, emailing, and mailing communications.
• Submit and track requests for chart uploads and assist with other administrative tasks to support Behavioral Health Division goals.
Quality Assurance:
• Participate in ICC chart audits, identify areas for improvement, and ensure documentation meets audit standards and criteria.
WORK SCHEDULE
This position is included in the County's alternate workweek program, working 40 hours during a standard workweek of Monday through Thursday (Fridays off). Specific hours of work will be discussed with the candidate selected for this position at the time an offer of employment is extended. The exact amount of on-site/telework and schedule will be discussed with the hiring manager during time of offer. Telework locations cannot be located outside of Oregon or Washington.
