PAY AND BENEFITS
Hourly Pay Range: $55.853432 - $71.063777
Salary offers will be made within the posted pay range and will be based on a candidate's experience (paid or unpaid) that is directly relevant to the position.

Employees must work a minimum of 30 hours per week to receive full benefits. Employees working 20 - 29 hours per week are eligible for medical benefits for themselves and family members. JOB DETAILS AND QUALIFICATIONS
We are looking for a Nurse Practitioner passionate about healthcare who is committed to working in a collaborative, team-based environment to join Clackamas County's Health clinic teams at our Sandy Health Clinic!
Clackamas Health Centers (CHC) provides fully integrated care to its patients in a dynamic, team-based environment. Our team of primary care providers are supported by a strong team of RNs, pharmacists (dispensing and clinical), behavioral health providers (including a strong primary care based MAT team), and dentists. We are looking for a family nurse practitioner passionate about caring for the underserved at our Sandy clinic. This position is a growth role- in other words, you get to build your own panel! This position will care for all ages, with more of a focus on adults. A strong interest in providing care for opioid use disorder is a plus.
Required Minimum Qualifications/ Transferrable Skills:*
• Licensed as a Registered Nurse by the State of Oregon
• Licensed and Board Certified as a Nurse Practitioner with prescriptive privileges (within 30 days of hire) by the State of Oregon
• DEA registration certificate with Schedule 2 - Schedule 5 privileges (within 30 days of hire)
• Must possess and maintain a Basic Life Support (BLS) certificate
Preferred Special Qualifications/ Transferrable Skills:*
• Experience working in an FQHC setting

TYPICAL TASKS
Primary Care & Patient Management
• Provide comprehensive primary care within an integrated healthcare system, ensuring high-quality patient care.
• Develop and implement personalized treatment plans based on patient needs and best practices.
• Deliver evidence-based preventive care, emphasizing early detection and disease prevention.
• Evaluate and recommend enhancements to healthcare services to improve patient outcomes.
• Participate in clinic operations, quality assurance initiatives, and continuous process improvements.
Diagnostics & Treatment
• Order, interpret, and analyze diagnostic tests and laboratory results to guide clinical decision-making.
• Diagnose and treat common acute and chronic conditions across various patient populations.
• Prescribe and administer medications in accordance with formulary guidelines and professional standards.
• Monitor patient responses to medications, provide education on potential risks and benefits, and adjust treatment plans as necessary.
Behavioral & Specialty Care Integration
• Integrate behavioral health services, including in-house psychiatric consultation and referral options, into patient care plans.
• Coordinate referrals to specialists, healthcare providers, and social service agencies to support patient care.
Pediatrics
• Perform comprehensive physical examinations for infants and children, assessing growth and development milestones.
• Diagnose and treat common pediatric conditions, ensuring timely and effective care.
• Educate and counsel parents on child health, development, and preventive care strategies.
• Determine and administer appropriate immunizations based on pediatric guidelines.
Women's Health & Obstetrics
• Conduct routine obstetric and gynecological examinations, diagnosing and managing common conditions.
• Provide patient-centered counseling and education on obstetrics, nutrition, reproductive health, contraception, sexually transmitted infections, parenting, family planning, and menopause.
Clinical Policy & Professional Development
• Contribute to the development, review, and implementation of clinical protocols, policies, and procedures.
• Engage in ongoing professional development, continuing education, and in-service training.
WORK SCHEDULE
This position works 24 hours during a standard workweek of Monday through Friday (3 days/week; 8 hours/day). Specific hours of work will be discussed with the candidate selected for this position at the time an offer of employment is extended. This position is designated as on-site/in-person. It is expected that the selected candidate will perform duties at the specified on-site work location.
