PAY AND BENEFITS
Case Manager Compensation Range:
Annual Pay Range: $57,351.80 - $72,266.58
Hourly Pay Range: $27.572979 - $34.743549
Salary offers will be made within the posted pay range and will be based on a candidate's experience (paid or unpaid) that is directly relevant to the position.

JOB DETAILS AND QUALIFICATIONS
Clackamas County's Health Centers is seeking a Case Manager to support and serve as a liaison between Health Centers and the Housing Division by working with individuals and families to ensure long-term housing sustainability. Specific case management focuses may include, but are not limited to, housing navigation, placement, maintenance and stability including paying rent, interacting positively with landlords and neighbors, managing behaviors that may accompany mental illness or substance use so they don't interfere with success in housing, developing crisis plans, connecting with appropriate community resources, and supporting clients' individual housing & health care goals. Case Managers provide skills training, case management, and care coordination. The Case Manager in this position must be skillful in interacting and engaging with a diverse client base while maintaining professionalism in a high need's environment.
Successful candidates will be able to provide culturally relevant services, have an anti-racist lens, will have completed some personal reflection and growth in their own equity journey, have a strong commitment to providing equitable services to the community, and demonstrating sensitivity to and respect for the diverse populations we serve. Case managers provide services to vulnerable individuals, many of whom may have limited resources. The person in this position must meet the requirements of a Qualified Mental Health Associate (QMHA) as outlined in the Oregon Administrative Rule and below.
Required Minimum Qualifications/ Transferrable Skills:*
• Qualified Mental Health Associate (QMHA) or (QMHA- R) as established by the Mental Health and Addiction Counseling Board of Oregon (MHACBO) as mandated by Oregan Administrative Rule at the time of hire.
A Qualified Mental Health Associate (QMHA) means a person delivering services under the direct supervision of a Qualified Mental Health Profession (QMHP) and meeting the following minimum qualifications as authorized by the Local Mental Health Authority (LMHA) or designee:
(a) Bachelor's degree in a behavioral sciences field; or
(b) A combination of at least three (3) years relevant work, education, training or experience.
• A minimum of three (3) years of related experience that would provide the required knowledge and skills to perform the responsibilities of this position
• Must possess and maintain a Basic Life Support (BLS) certificate
• Experience, knowledge, and commitment to serving a diverse population and promoting equity, diversity and inclusion
Preferred Special Qualifications/ Transferrable Skills:*
• Fluency in Spanish/English, both oral and written (bilingual skills to be validated/tested at time of interview and/or selection)
• Experience working in a primary care or behavioral health care setting
• Experience providing housing services and support to vulnerable clients
• Experience providing case management services for clients diagnosed with severe and persistent mental illness (SPMI) and/or substance use disorders (SUD)

TYPICAL TASKS:
Duties may include but are not limited to the following:
• Works with clients to develop individualized housing plans; assist clients with housing applications, completes supportive and subsidized housing paperwork, assist with navigating housing options and placement, including advocating for clients with prospective landlords. Monitors and evaluations each person's progression through their housing and behavioral health plan; develops corrective action revisions to the plan as needed.
• Provides case management, skills training and care coordination services for individuals/families who are receiving behavioral health and housing supports; develops and coordinates resources to provide clients and potential clients with needed services; community outreach and coordination with service providers. Utilize agency approved Evidence Base Practices. Utilize outcome measures identified by Clackamas County Behavioral Health Center to monitor client progress in their recovery.
• Coordinates with health care providers, social service personnel, and other agencies to obtain housing supports and services; provides information and referral services to clients and families; advocates for individuals/families with other agencies for services. Identifies and collaborates across programs and organization to serve the house needs of the community.
• Determines housing, health and social service needs of clients; interviews applicants, recipients, relatives, guardians, care providers, and other involved parties; conducts comprehensive needs assessments, including psychosocial, financial, and physical factors; evaluates functional impairment of clients. Make mandatory reports as defined by state law and agency policy.
• Provide other duties as assigned.
REQUIRED KNOWLEDGE AND SKILLS
Working knowledge of: Methods and techniques of case management; social, health care, financial and other community resources and agencies; principles and techniques of interviewing, counseling and crisis intervention; laws, rules and regulations applicable to government programs and funding guidelines, including Title XIX; principles and techniques human development and behavior in assigned area, such as geriatrics, physical disabilities, developmental disabilities, chronic mental illness, drug and alcohol abuse, or economically disadvantaged; office equipment, including personal computers and software programs.
Some knowledge of: Policies and procedures related to protective services, including client rights, hearings procedures and legal terminology; laws, rules and regulations applicable to involuntary commitment, protective services, guardianship, conservatorship, informed medical consent, confidentiality, and facility certification and regulation; medical criteria for levels of care, disease processes and medical terminology.
Skill to: Gather information through research and interviews with applicants, clients, families, care providers and others; apply comprehensive social service evaluation and assessment methods; develop and implement appropriate case plans for clients with diverse needs and diverse settings; identify community/client needs and develop programs and resources to meet those needs; interpret and apply federal, state and local laws, rules, regulations and policies; analyze relevant data and authorize expenditure of public funds in compliance with guidelines; compute payments for clients/program related expenses; maintain accurate and concise records; communicate effectively, both orally and in writing; establish and maintain effective working relationships with clients, families, care providers, community resources, other public and private agencies and County employees; operate computer software and other office equipment.
WORK SCHEDULE
This position works 40 hours during a standard workweek of Monday through Friday.
Specific hours of work will be discussed with the candidate selected for this position at the time an offer of employment is extended.

The exact amount of on-site/telework and schedule will be discussed with the hiring manager during time of offer. Telework locations cannot be located outside of Oregon or Washington.
