PAY AND BENEFITS
Annual Pay Range: $93,209.97 - $125,833.42
Hourly Pay Range: $44.812484 - $60.496838
The classification for this position is Quality & Data Supervisor, with a working title of Population Health Manager.
Salary offers will be made within the posted pay range and will be based on a candidate's experience (paid or unpaid) that is directly relevant to the position.

JOB DETAILS AND QUALIFICATIONS
Clackamas County Health Centers is looking for a team-oriented, visionary, and strategic leader who is passionate about the quality, health and well-being of the community. The role of Population Health Manager (classified as Quality & Data Supervisor) with Clackamas County Health Centers is a pivotal position that requires a dynamic and dedicated individual. This leader will be at the forefront of shaping the future of healthcare within the community, driving initiatives that directly impact the lives of the community. The ideal candidate will possess a deep understanding of population health management and quality improvement methodologies. They will be adept at navigating the complexities of healthcare systems, fostering an environment of continuous improvement, and championing policies that promote equitable health outcomes.
The responsibilities of this role are comprehensive and multifaceted. The manager will oversee the Population Health team and Quality Improvement Analysts, ensuring that the programs and activities align with the overarching goals of the Health Centers. They will be instrumental in developing strategies that enhance clinical quality, streamline care transitions, and address health disparities. Moreover, the manager will play a crucial role in advancing value-based care initiatives, which are essential for the sustainability and efficiency of healthcare delivery.
In addition to program development and implementation, the Population Health Manager will manage and mentor staff involved in population health efforts. This includes providing training, guidance, and support to build a competent and motivated team. The supervisor will also be responsible for monitoring and reporting on the performance of quality initiatives, providing insights and recommendations to the clinical, quality, and operational leadership.
This candidate selected for this role, will not only be skilled in data analysis and quality improvement but also exhibit strong interpersonal and communication skills. The manager must be able to collaborate effectively with various stakeholders, including healthcare providers, administrative staff, and community partners, to foster a holistic approach to health and wellness.
The Population Health Manager will be a key driver in ensuring that Clackamas County Health Centers delivers high-quality care that is accessible, effective, and responsive to the needs of the community. The individual in this role will have the opportunity to make a significant impact, shaping health policies and practices that will resonate and benefit the well-being of the Clackamas County community.
The Population Health Manager is a member of the Quality Improvement and Quality Assurance Team, and reports to the Deputy Medical Director.
Required Minimum Qualifications/ Transferrable Skills:*
• A minimum of five (5) years of related experience that would provide the required knowledge and skills to perform the responsibilities of this position
• at least two (2) years to include lead, supervisory, or project management experience
• Experience in quality, performance improvement, or related field in healthcare
• Experience with population health approaches to healthcare delivery
Preferred Special Qualifications/ Transferrable Skills:*
• Experience in Federally Qualified Health Center (FQHC) or Community Health environments
• Lean or Six Sigma experience
• Clinical background as an RN, behaviorist or other clinical role

TYPICAL TASKS
• Supervise and guide population health staff in executing transitional care management, outreach, and care coordination to bridge care gaps and ensure smooth care transitions.
• Lead quality improvement (QI) efforts by employing evidence-based strategies to enhance clinical and operational performance metrics.
• Collaborate with the Quality Assurance Manager, Lead Data Analyst, and Lead EHR Systems Analyst to leverage data reports and dashboards for tracking and enhancing population health outcomes.
• Implement data integration tools like Point Click Care, CareMessage, and other platforms for effective patient communication and care gap identification.
• Utilize quality and process improvement methodologies, such as Plan-Do-Study-Act (PDSA) cycles, to optimize daily operations and team performance.
• Engage in clinical improvement activities and audits to advance team performance and meet health center standards.
• Oversee population health projects, ensuring compliance with grant and regulatory demands, and focus on enhancing value-based care and population health initiatives.
• Analyze data to fulfill financial and clinical contractual obligations, manage budgets, and maintain adherence to reporting requirements.
• Communication and Collaboration:
• Establish and maintain liaison relationships with key personnel in similar roles across local programs and agencies.
• Engage with senior staff, Clinic Managers, and Supervisors to foster a collaborative environment.
• Prepare comprehensive reports that summarize clinical and operational performance metrics.
• Present findings and insights to both internal and external stakeholders to inform decision-making.
• Participate actively in staff committees, teams, and workgroups to contribute to organizational goals.
• Staff Development:
• Design and implement training programs for staff focused on population health initiatives and associated workflows.
• Facilitate onboarding processes to ensure new staff members are well-versed in program objectives and procedures.
• Monitor and evaluate the effectiveness of training and development programs to continuously improve staff competencies.
WORK SCHEDULE
This position is included in the County's alternate workweek program, working 40 hours during a standard workweek of Monday through Thursday (Fridays off).
Specific hours of work will be discussed with the candidate selected for this position at the time an offer of employment is extended.
This position is eligible for hybrid on-site/telework subject to the https://dochub.clackamas.us/documents/drupal/9065b5fb-2237-47ce-9534-e7c8c40fc2bc and based on the Department's business needs. The exact amount of on-site/telework and schedule will be discussed with the hiring manager during time of offer. Telework locations cannot be located outside of Oregon or Washington. Washington residents must perform 50% of their time on-site at Clackamas County.
