PAY AND BENEFITS
Annual Pay Range: $55,971.77 - $70,527.67
Hourly Pay Range: $28.703471 - $36.168035
Salary offers will be made within the posted pay range and will be based on a candidate's experience (paid or unpaid) that is directly relevant to the position.

JOB DETAILS AND QUALIFICATIONS
Clackamas County's Health Centers is seeking a Case Manager to join and support the Mental Health Court Program. This position will provide case management, skills training services, and care coordination to clients within our behavioral health program. The Case Manager in this position must be skillful in interacting and engaging with a diverse client base while maintaining professionalism in a stressful environment. The position will include in-home visits and transporting individuals as part of a service delivery model.
Successful candidates will be able to provide culturally competent services, have an anti-racist lens, will have completed some personal reflection and growth in their own equity journey, have a strong commitment to providing equitable services to the community, and demonstrating sensitivity to and respect for the diverse populations we serve. Case managers provide services to vulnerable individuals, many of whom may have limited resources. Professionals in these positions should thrive in a setting where they will work independently as well as in a team environment.

Required Minimum Qualifications/ Transferrable Skills:*
• Qualified Mental Health Associate (QMHA) or (QMHA- R) as established by the Mental Health and Addiction Counseling Board of Oregon (MHACBO) at the time of hire.
A Qualified Mental Health Associate (QMHA) means a person delivering services under the direct supervision of a Qualified Mental Health Profession (QMHP) and meeting the following minimum qualifications as authorized by the Local Mental Health Authority (LMHA) or designee:
(a) Bachelor's degree in a behavioral sciences field; or
(b) A combination of at least three years relevant work, education, training or experience.
• A minimum of three (3) years of related experience that would provide the required knowledge and skills to perform the responsibilities of this position
• Must possess and maintain a Basic Life Support (BLS) certificate
• Experience working in a primary care or behavioral health care setting
• Experience providing case management services for clients diagnosed with severe and persistent mental illness (SPMI) and/or substance use disorders (SUD)
• Experience, knowledge and commitment to serving a diverse population and promoting equity, diversity and inclusion
Preferred Special Qualifications/ Transferrable Skills:*
• Experience working on a Treatment Court Team or in a similar program providing behavioral health case management to clients
• Experience working with justice involved clients or court ordered clients
• Possession of a current CADC or CADC-R certification

TYPICAL TASKS
Client Assessment & Planning
• Conducts interviews with clients, families, guardians, and care providers.
• Performs comprehensive needs assessments (psychosocial, financial, physical).
• Evaluates clients' functional impairments.
• Develops and implements individualized financial and service plans.
• Determines appropriate treatment and care services.
• Matches client needs with suitable providers and sets payment levels.
Service Coordination, Advocacy & Support
• Coordinates services with healthcare providers, social services, and agencies.
• Provides clients and families with information and referral services.
• Regularly reviews and updates case plans and eligibility.
• Advocates for clients with agencies, care providers, and institutions.
• Responds to emergencies and mediates problem resolution.
• Offers counseling and training on care plans, life skills, personal goals, financial planning, vocational readiness, and medication management.
Monitoring & Home Visits
• Monitors client progress and ensures case plan implementation.
• Conducts home, hospital, and residential facility visits.
• Observes and documents changes in client condition or care deficiencies.
• Reports and may investigate abuse or neglect.
Resource Development & Outreach
• Develops resources and community support services.
• Initiates support groups and volunteer programs.
• Markets programs and recruits clients, providers, and volunteers.
• Prepares grant applications; monitors grant funds.
• Negotiates and oversees service provider contracts.
Documentation & Reporting
• Prepares case summaries and documents services provided.
• Maintains case statistics and records.
• Submits and modifies documentation for payments and benefits.
• Prepares materials for hearings and legal proceedings.
WORK SCHEDULE
This position works 40 hours during a standard workweek of Monday through Friday (Ideal work schedule: 8:30 AM to 5:00 PM).
Specific hours of work will be discussed with the candidate selected for this position at the time an offer of employment is extended. The exact amount of on-site/telework and schedule will be discussed with the hiring manager during time of offer. Telework locations cannot be located outside of Oregon or Washington. Washington residents must perform 50% of their time on-site at Clackamas County.
